450 Smart car on fire
This unusual incident has happened a few time in Europe. The woman in this fire got pulled out just in time and is OK.
This is the latest incident in the UK, and can happen on the 450 City car (fortwo) if people are not used to it, as when they go to fill up with petrol they can with force, pull the air inlet cover off as its round, just like the petrol filler on the other side. They then fill the air filter and leak petrol over the engine and can cause a fire, when engine is turned on.
This is impossible to do on the 451, as the air inlet is like a thin half moon shape, and looks nothing like the round fuel filler cap on the other side.

Never heard of this issue but it's a serious design flaw. How many reported cases have there been of this happening exactly?

It is hard to say how many people do the above stupid thing, but it has happened a number of times due to the 450 design, and an owner just getting the car and not being told where the correct fuel filler cap is..
If you google 'Smart car on fire', you see a lot of burned out Smarts, but unless you check each story individually to see if the car was getting re-fueled in a garage, it could be down to the above, or electrical fault's, or extreme overheating burning something, or car accidents, or vandalism.
At least on the 451 and 453 Fortwo's, you instantly know where the fuel filler cap is
